What a decode looks like

Synthetic example. Every note points at the exact line it is about.

Sample explanation of benefits · Northgate Health Plan (synthetic)
  1. Date of service 03/11 · Northgate Imaging Center
  2. Billed amount: $2,340.00 · Plan paid: $0.00
  3. Denial code CO-197: precertification/authorization absent
  4. Patient responsibility: $2,340.00
  5. You may appeal within 180 days of the date of this notice.

Why this is hard

  • Codes instead of words

Procedure codes, "adjustments" and "patient responsibility" on a medical bill, or "one-time charges" on a phone bill, don't say what you're paying for.

  • The denial reason is one line

"Not medically necessary" or "authorization absent", a deadline to appeal, and no explanation of what to send.

  • Writing the letter takes an evening

Even when something looks wrong, a clear complaint or appeal that cites the right lines is hard to start.

What you get

  • Every charge or denial reason in plain English, quoted from the page
  • Amounts and deadlines pulled out so they're easy to see
  • Lines worth asking about, such as duplicate charges or fees you didn't agree to
  • An editable complaint or appeal letter template that cites the lines it refers to

What it is not

An explanation of your bill or letter plus editable templates. It isn't legal, financial or medical advice, and it doesn't decide whether a charge is wrong or how an appeal will turn out.

Credit packs from $4.99 for 5 credits (plus applicable sales tax).

Questions

Which documents work?

Medical bills, explanations of benefits, insurance claim denials, and phone, internet or utility bills. In Canada, the telecom complaint template follows the usual path: your provider first, then the CCTS.

Why not just ask ChatGPT?

You can, and for a short bill it may be enough. What we add is a fixed structure: every charge or reason in the same format, each quoted from the page so you can check it, deadlines pulled out, and a letter template that cites those lines. There's no prompt to write, and uploaded files are deleted within 24 hours unless you choose to save them. Whether that's worth 1 credit is what we're asking before we build it.

Will my complaint or appeal succeed?

We can't predict that. We explain what the document says and give you a clear letter to start from; the decision is your provider's or insurer's.
